Directive -> plugin (#3363)
Caught my eye, we name things directive still, esp when talking about
the prometheus *plugin*. Rename everything that needs to be plugin to
'plugin'. Also make sure Metrics is a H2 section (not H1).

62317c3c14 update doc to not use the root zone for everything (#3288)
Update all documentation in the tree to use example.org as an example
configuration (in so far possible). As to get out of the just use "."
and fallthrough and things would be fine.

plugin/hosts: create inline map in setup (#3071)
* plugin/hosts: create inline map in setup

The inline map wasn't create in the setup.go fuction leading to a crash,
which is masked by a recover (but leads to a SERVFAIL, and not logging
the request).

Various other simplifications.

host plugin could use some test that actually cover these edgecases.

89fa9bc61e plugin/host: don't append the names when reparsing hosts file (#3045)
The host plugin kept on adding entries instead of overwriting. Split the
inline cache off from the /etc/hosts file cache and clear /etc/hosts
file cache and re-parsing.

A bunch of other cleanup as well. Use functions defined in the plugin
package, don't re-parse strings if you don't have to and use To4() to
check the family for IP addresses. Fix all test cases a carried entries
are always fqdn-ed. Various smaller cleanup in unnessacry constants.

* plugin/hosts provide more configuration flexibility

This patch adds few features to the host plugin
 * no-reverse (both as first argument on the plugin line and inline)
   disable the automatic generation of reserve entries for hosts
 * ttl <duration> (inline only atm)
   allows to change the default ttl (default 5 minutes)
 * reload <duration> (inline only atm)
   allows to change the reloading interval (default 5s)

* plugin/hosts remove superfluous parameters to parse

4083852b70 Remove trailing whitespace (#1955)
Prevent future; "remove trailing whitespace" PR, but adding a simple
presubmit that checks for this.

This presubmit flagged quite some offenders, remove all trailing
whitespace from. Apart from that there aren't any other changes.

return an error for multiple use of some plugins (#1559)
* plugins: Return error for multiple use of some

Return plugin.ErrOnce when a plugin that doesn't support it, is called
mutliple times.

This now adds it for: cache, dnssec, errors, forward, hosts, nsid.
And changes it slightly in kubernetes, pprof, reload, root.

Add pkg/fall for Fallthrough (#1355)
* Add pkg/fall for Fallthrough

Move this into it's own package to facilitate tests. Important bug
was fixed: make the names fully qualified.

Add fall package to hosts, reverse, etcd, and fix kubernetes and any
tests. The k8s tests are still as-is, might need a future cleanup.

Generate manual pages from the README and extend README with Name and

The generation requires 'ronn' which may not be available. Just check in

7109c6715c Add inline support for middleware/hosts (#1072)
This fix add inline support for middleware/hosts so that
it is possible to specify hosts file insides the Corefile:
```
hosts inline example.org {
    10.0.0.1 example.org
    fallthrough
}
```

This fix fixes 999.
