The specifics of how the authorization for a request is propagated
through the registry app are private implementation details. Hide those
details from outsiders so they can be changed as needed without fear of
breaking third-party code. Move the utilities for attaching a request's
authorization status to its context and retrieving it from the context
into the registry/handlers package as unexported symbols.

Our context package predates the establishment of current best practices
regarding context usage and it shows. It encourages bad practices such
as using contexts to propagate non-request-scoped values like the
application version and using string-typed keys for context values. Move
the package internal to remove it from the API surface of
distribution/v3@v3.0.0 so we are free to iterate on it without being
constrained by compatibility.

Go 1.13 and up enforce import paths to be versioned if a project
contains a go.mod and has released v2 or up.
The current v2.x branches (and releases) do not yet have a go.mod,
and therefore are still allowed to be imported with a non-versioned
import path (go modules add a `+incompatible` annotation in that case).
However, now that this project has a `go.mod` file, incompatible
import paths will not be accepted by go modules, and attempting
to use code from this repository will fail.
This patch uses `v3` for the import-paths (not `v2`), because changing
import paths itself is a breaking change, which means that the
next release should increment the "major" version to comply with
SemVer (as go modules dictate).

context.App.repoRemover is single registry instance stored throughout
app run. It was wrapped in another remover when processing each request.
This remover happened to be remover got from previous request. This way
every remover created was stored in infinite linked list causing memory
leak. Fixing it by storing the wrapped remover inside the request context
which will get gced when request context is gced. This was introduced in
PR #2648.

Back in the before time, the best practices surrounding usage of Context
weren't quite worked out. We defined our own type to make usage easier.
As this packaged was used elsewhere, it make it more and more
challenging to integrate with the forked `Context` type. Now that it is
available in the standard library, we can just use that one directly.
To make usage more consistent, we now use `dcontext` when referring to
the distribution context package.

Ensure that the status is logged in the context by instantiating before the
request is routed to handlers. While this requires some level of hacking to
acheive, the result is that the context value of "http.request.status" is as
accurate as possible for each request.

This changeset implements immutable manifest references via the HTTP API. Most
of the changes follow from modifications to ManifestService. Once updates were
made across the repo to implement these changes, the http handlers were change
accordingly. The new methods on ManifestService will be broken out into a
tagging service in a later PR.
Unfortunately, due to complexities around managing the manifest tag index in an
eventually consistent manner, direct deletes of manifests have been disabled.

After consideration, it has been decided that the interfaces defined in the
storage package provide a good base for interacting with various registry
instances. Whether interacting with a remote API or a local, on-disk registry,
these types have proved flexible. By moving them here, they can become the
central components of interacting with distribution components.

The goal is to free up the distribution/registry package to include common
registry types. This moves the webapp definitions out of the way to allow for
this change in the future.
