gofumpt (https://github.com/mvdan/gofumpt) provides a supserset of `gofmt` / `go fmt`,
and addresses various formatting issues that linters may be checking for.
We can consider enabling the `gofumpt` linter to verify the formatting in CI, although
not every developer may have it installed, so for now this runs it once to get formatting
in shape.

Go 1.13 and up enforce import paths to be versioned if a project
contains a go.mod and has released v2 or up.
The current v2.x branches (and releases) do not yet have a go.mod,
and therefore are still allowed to be imported with a non-versioned
import path (go modules add a `+incompatible` annotation in that case).
However, now that this project has a `go.mod` file, incompatible
import paths will not be accepted by go modules, and attempting
to use code from this repository will fail.
This patch uses `v3` for the import-paths (not `v2`), because changing
import paths itself is a breaking change, which means that the
next release should increment the "major" version to comply with
SemVer (as go modules dictate).

If htpasswd authentication option is configured but the htpasswd file is
missing, populate it with a default user and automatically generated
password.
The password will be printed to stdout.

Back in the before time, the best practices surrounding usage of Context
weren't quite worked out. We defined our own type to make usage easier.
As this packaged was used elsewhere, it make it more and more
challenging to integrate with the forked `Context` type. Now that it is
available in the standard library, we can just use that one directly.
To make usage more consistent, we now use `dcontext` when referring to
the distribution context package.

By default all repository classes are allowed.

The token auth package logs JWT validation and verification failures at
the `error` level. But from the server's perspective, these aren't
errors. They're the expected response to bad input. Logging them at
the `info` level better reflects that distinction.

Adds a constant leeway (60 seconds) to the nbf and exp claim check to
account for clock skew between the registry servers and the
authentication server that generated the JWT.
The leeway of 60 seconds is a bit arbitrary but based on the RFC
recommendation and hub.docker.com logs/metrics where we don't see
drifts of more than a second on our servers running ntpd.
I didn't attempt to make the leeway configurable as it would add extra
complexity to the PR and I am not sure how Distribution prefer to
handle runtime flags like that.
Also, I am simplifying the exp and nbf check for readability as the
previous `NOT (A AND B)` with cmp operators was not very friendly.

Since RawMessage json receivers take a pointer type, the Header structure should use points in order to call the json.RawMessage marshal and unmarshal functions

Previously, this error message would stringify as a pointer address,
which isn't particularly helpful.
This change breaks out the elements of the challenge object such that
the error is appropriately represented.

This removes the erroneous http.Handler interface in favor a simple SetHeaders
method that only operattes on the response. Several unnecessary uses of pointer
types were also fixed up.

Per the comment on line 498, this moves the logic of setting the http
status code into the serveJSON func, leaving the auth.Challenge.ServeHTTP()
func to just set the auth challenge header.

After consideration, the basic authentication implementation has been
simplified to only support bcrypt entries in an htpasswd file. This greatly
increases the security of the implementation by reducing the possibility of
timing attacks and other problems trying to detect the password hash type.
Also, the htpasswd file is only parsed at startup, ensuring that the file can
be edited and not effect ongoing requests. Newly added passwords take effect on
restart. Subsequently, password hash entries are now stored in a map.
Test cases have been modified accordingly.

This change refactors the basic authentication implementation to better follow
Go coding standards. Many types are no longer exported. The parser is now a
separate function from the authentication code. The standard functions
(*http.Request).BasicAuth/SetBasicAuth are now used where appropriate.
